If you’re applying for permanent residency in the United States, completing Form I-693 is a required step — and it must be done by a USCIS-designated civil surgeon. The I-693 Form is used to report examination results and an appointment consists of health questions, brief physical and mental health screening, blood test for tuberculosis (TB), and for applicants 14 years and older a blood test for syphilis and urine test for gonorrhea. Immunizations are also required and are based on your age and vaccination history. Depending on the applicant’s personal medical history and other health factors, additional screening or testing may be required.
Be sure to bring the following for your appointment:
- Vaccination records
- Prior TB treatment records or chest X-rays, if applicable
- Government-issued photo ID
- Completed Part 1 of Form I-693
Your completed I-693 will be provided in a sealed envelope for Form I-485 submission and an unsealed copy for your personal records.
Your sealed I-693 is typically ready within 10 to 14 days of your first visit. You’ll receive a sealed envelope for USCIS submission and an unsealed personal copy. Payment is due at each point of service; cash and major credit cards accepted.
