We perform the complete immigration medical examination required by USCIS for I-485 form submission. This includes a comprehensive physical exam by a USCIS-designated civil surgeon, TB blood testing and communicable disease screening, a mental and physical health review, a substance use assessment, and a full vaccination evaluation. All USCIS-required vaccines are available at Passport Health locations including MMR, Tdap, hepatitis A, hepatitis B, influenza, varicella, meningococcal, pneumococcal, polio, Hib, and rotavirus.
Complete Part 1 of Form I-693 before your first visit. Bring your vaccination records, a government-issued photo ID, and any TB treatment documentation, if you have been treated for it in the past. If records are incomplete, we provide vaccinations and titer testing.
