When applying for a visa, green card or U.S. citizenship, an immigration medical exam, the I-693, is required to be completed by a USCIS-designated civil surgeon. Our staff is authorized and able to perform a thorough medical review, provide required immunization and complete the I-693 with you.

The full I-693 exam includes physical examination, communicable disease lab work, mental and physical health screening, substance use assessment, and vaccination review. Required vaccines including MMR, Tdap, hepatitis A, hepatitis B, and others are all available on-site. Sealed I-693 typically ready within 10 to 14 days.
Official acceptable forms of identification ( i.e. passport, driver’s license, or other official identification), basic demographic & personal information, vaccination records, and signatures are all required for your first visit. As part of the I-693 requirement, we will complete all components to include: physical examination, psychological evaluation, required titers (immunity levels), and testing for tuberculosis and communicable diseases, as well as all USCIS required vaccinations.
Upon process completion, your approved, signed and sealed I-693 Report of Medical Examination and Vaccination Records documents will be given to you. This package must be delivered to the USCIS as part of the application procedure and cannot be opened. You will also be provided with an identical set of these documents for your records, that have not been sealed.
If there are no complications, you can expect to receive paperwork at your physical, which is generally 10-14 days after your initial appointment.
Pricing can vary depending on an individual’s age and needs. Payment is due at the time of service for all fees, lab work, x-rays, etc. If the patient needs additional lab tests or vaccinations, that will be an additional charge at the time of the in-person physical.
