When applying for a visa, green card or U.S. citizenship, an immigration medical exam, the I-693, is required to be completed by a USCIS-designated civil surgeon. Our staff is authorized and able to perform a thorough medical review, provide required immunization and complete the I-693 with you.

Your immigration physical involves two appointments. The first is a nursing visit covering medical history, lab work, and any required vaccinations — including MMR, Tdap, polio, and others available on-site. The second is your civil surgeon appointment for the physical exam and I-693 certification. Your sealed I-693 envelope is typically ready within 10 to 14 days of your first visit.
Be sure to bring the following for your appointment:
- Photo identification
- Passport and/or visa
- Hospital or clinic records
- Prior chest x-rays and any other diagnostic tests
- Vaccination records or titers
If there are no complications, you can expect to receive paperwork at your physical, which is generally 10-14 days after your initial appointment.
Pricing can vary depending on an individual’s age and needs. Payment is due at the time of service for all fees, lab work, x-rays, etc. If the patient needs additional lab tests or vaccinations, that will be an additional charge at the time of the in-person physical.
