Passport Health uses civil surgeons for all immigration physicals. The following locations provide immigration physical examinations in the Tampa region:

Regardless of immigration category, the exam covers the same required components: physical examination, communicable disease lab work (TB, syphilis, gonorrhea for ages 15+), mental and physical health screening, substance use assessment, and a full vaccination review. All required vaccines — including MMR, Tdap, hepatitis A, and others — are available on-site.
Be sure to bring the follow for your immigration medical examination:
- Personal vaccination records (in any format or language)
- Photo ID for adults; passport or birth certificate for children
- TB treatment records or prior chest X-rays, if applicable
- Completed Part 1 of Form I-693 (parents complete this for minors)
Upon completion, you receive a sealed I-693 envelope for USCIS submission and an unsealed personal copy. The civil surgeon’s signature is valid for 60 days from the date of signing. USCIS must issue a decision within two years of your I-693 submission. All fees are due at time of service; we accept cash and major credit cards.
